The Social Worker liaises with the treatment team, the family and the available community resources.<\/p>\n<p>The Social Worker\u2019s role is to:<\/p>\n<ul class='list-reset-disc'>\n<li>educate the clinical team on the patient\u2019s environment<\/li>\n<li>encourage family and friends to take part in the rehabilitation process<\/li>\n<li>organize the discharge planning based on the patient\u2019s needs, in collaboration with friends and family<\/li>\n<li>liaise, if necessary, with various institutions and community resources that may be involved in the reintegration process at home or elsewhere<\/li>\n<li>find out which programs the patient can use and has access to<\/li>\n<li>provide housing assistance, if necessary<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>At Villa Medica, the Social Worker\u2019s role is to facilitate the patient\u2019s reintegration into society, with all of the challenges presented by their illness or disability.
