Medical archives

The health and performance information management team is the loyal guardian who ensures the confidentiality of your personal data.

At your request, they send your information to the various professionals who provide your medical follow-up upon discharge from the Villa Medica rehabilitation hospital.

The medical record is the witness of your clinical condition and of your follow-up by our team of stakeholders. The information it contains is yours and you have the right to access it upon request.

Would you like a copy of your medical file?

Access to the medical file is subject to the Law of the Services de Santé et des Services Sociaux. It is confidential and no one can access it without the consent of the user. To access your medical file, you must submit a written request, which must contain the following information:

  • Your last name and first name at birth;
  • Your date of birth;o Your health insurance number,
  • The nature of the documents requested;
  • The name and address of the recipient.

You must sign and date your request (only handwritten signatures will be accepted). Please note that depending on the volume of documents requested, fees may apply.

No request will be accepted by telephone. If fees apply, they must be paid before the documents are copied.

Want to see your medical records?

Any consultation of medical records is done on site with the presence of a medical archivist. Upon receipt of a written and signed request from you, we will contact you to set up an appointment. Consultations are done only during the week, between 8 a.m. and 4 p.m. excluding holidays.

Want to get a certificate of hospitalization?

The certificate of hospitalization is a document made upon request. It contains the dates of your hospitalization, the name of the doctor who requested your rehabilitation transfer as well as that of Villa Medica, the type of treatment received and the identification of our establishment. If you think you need this document, send us a written request. Fees are applicable to this request.

Want to obtain a certificate for your employer, confirming the presence of a relative in the hospital?

As with the certificate of hospitalization, you must send us a written request and sign the person hospitalized. This document will attest to the presence of the latter at the Villa Medica rehabilitation hospital as well as the name of the attending physician.
