Measure, evaluate, improve: share information
Via a clinical-research exchange platform, we, at the encephalopathy program of the IRDPQ-CIUSSSCN, made the choice of standardized clinical measures to be used to assess users and monitor the clientele. Over time, the analysis of these cumulative measurements in a database has enabled us to carry out various continuous improvement projects, leads us to a better knowledge of the progress of users and helps to compare with scientific writings. Currently, the data helps us to work on the actions of the MSSS AVC 2016-2018 plan.

The use of first aid to users as information gathering for occupational therapists
When users are admitted to a rehabilitation centre, a series of professional evaluations follow during the first days, even weeks. But how to use the work of care attendants to collect permanent data and information to allow the beginning of the development of clinical judgment by the occupational therapist (his analysis) and thus adjust and prioritize his involvement. This project was launched four years ago at the Marie-Clarac hospital by training two beneficiary attendants who provide first aid in hygiene, transfer and dressing.

The systematic use of the Mayo-Portland Adaptability Inventory (MPAI-4) in several rehabilitation establishments in the continuum of traumatology services can support the teams’ clinical decision-making and is in the process of generating a critical mass of data, unprecedented in Quebec. This information will allow us to better understand the functional evolution of users through the phases of the continuum and to take a look at the effectiveness of rehabilitation in order to optimize it.

The ministerial orientations aim to establish a continuum of services for stroke survivors, from the hyperacute phase to integration into the community. The assisted early discharge program is introduced within the continuum and consists of intensive functional rehabilitation services at home for clients who have had a stroke with mild to moderate impairment. The CIUSSS Centre-Ouest-de-l’Île-de-Montréal and Centre-Sud-de-l’Île-de-Montréal began implementing these services in 2017 in order to improve the flow of users through the Montreal network and reduce the average length of stay of this clientele in acute care and internal rehabilitation and, by extension, in the emergency room.
The CIUSSS Centre-Ouest-de-l’Île-de-Montréal and Centre-Sud-de-l’Île-de-Montréal began implementing these services in 2017 in order to improve the flow of users through the Montreal network and reduce the average length of stay of this clientele in acute care and internal rehabilitation and, by extension, in the emergency room.
This workshop will be of interest to participants who would like to implement the program in their organization. The speakers will share their successes and challenges during implementation.

Home risks: evaluation grid and intervention tools
The delivery of services at home presents challenges for workers, users and loved ones since each home is unique and often poorly adapted to the needs of individuals with a loss of autonomy or with disabilities and handicaps.
The purpose of the CISSS de l’Outaouais home risk factor observation grid is to establish safe strategies for users in order to promote safe home care based on a grid shared with the user. and his family and the various stakeholders involved. These observations are then recorded, when required, in the intervention plans and are subject to follow-up within the plan. A regular reassessment of risk factors at home promotes the improvement of the quality of care and services to the user and empowers the latter in the search for a strategy to promote his maintenance in his living environment.

The research project aims to assess the clinical and socio-economic effectiveness of kinematic knee imaging and the My Arthrosis program in the first-line management of osteoarthritis. 2000 patients who suffer from painful osteoarthritis will be recruited in more than 100 clinics and followed over a period of 24 months for their clinical condition and their private and public expenses associated with this disease .

This residence welcomes clients on PD, with cognitive impairment secondary to TBI or stroke. Catastrophizing emerged as the most important psychological factor in predicting poor recovery from musculoskeletal injury. Significant progress has been made in developing tools to assess it. However, the development and implementation of interventions to reduce catastrophic thinking has progressed at a much slower pace. This presentation aims to equip rehabilitation workers with the tools necessary to effectively detect and intervene in catastrophic thinking. The skills acquired as a result of this presentation could contribute to better clinical results for clients who present a profile with psychological risks.

User engagement: from partnership to referral
At Villa Medica, the user undertakes to collaborate in his care by first being a partner in his own intervention plan. Indeed, all users actively participate in the development of their P3I by their presence at the first interdisciplinary meeting which brings together all their stakeholders. In addition, considering it important to involve and list as many experiences as possible from users, Villa Medica has succeeded in integrating at the very heart of several of its strategic and tactical committees, one or more users who come to feed the exchanges and serve thus a reference in terms of customer experience.

Sensory rehabilitation of neuropathic pain
Neuropathic pain has a major impact on lifestyle and is a major challenge in rehabilitation. The sensory rehabilitation method developed by Claude Spicher and based on the neuroplasticity of the somatosensory system is used to treat this pain. This workshop offers an overview of the basic principles of the method. The main assessments and treatment modalities for mechanical allodynia and hypoesthesia will be demonstrated.
